Who this policy applies to
This policy applies to personal information we collect from people who contact us, enquire about services, complete intake paperwork, are referred to us by family members or professional contacts, apply for jobs, or otherwise deal with SunState Care.
What information we collect
Depending on the circumstances, we may collect your name, contact details, enquiry details, service preferences, referral information, job application details, and information about support needs, health, disability or care requirements where that information is relevant and provided to us.
How we collect information
We collect personal information directly from you when you contact us by website form, phone or email, when you complete intake forms, and when you submit a job application. We may also receive information from family members, support coordinators, plan managers or other referrers where you have asked them to contact us or where it is otherwise lawful to do so.
Why we collect, use and hold information
We collect, use and hold personal information to respond to enquiries, assess whether we can provide services, deliver supports, communicate with you or your representatives, manage recruitment, maintain our records, and meet legal, safety, insurance and compliance obligations.
Disclosure of personal information
We may disclose personal information where reasonably necessary to provide services, respond to enquiries, operate our business, or comply with the law. This can include disclosures to referrers, your authorised representatives, professional advisers, insurers, regulators, courts or tribunals, and Australian service providers who help us run our systems and communications. We store personal information using systems and service providers based in Australia. We take reasonable steps to protect the information we hold from misuse, interference, loss, unauthorised access, modification and disclosure. At the date of this policy, we do not intend to disclose personal information to overseas recipients.

When we no longer need the information, we will take reasonable steps to destroy it or de-identify it, unless we are required or authorised by law to retain it.
